Pain Control post knee surgery? Post knee surgery (conformis iForma implant) for category 4 osteoarthritis. I have been prescribed : Aspirin (Dispersible) - 150mg once daily Omeprazole – 20mg once daily to be taken with the Aspirin Ibuprofen 400mg every 8 hrs Tramadol 100mg every 6 hrs Paracetamol 1000mg every 6hrs I did try leaving off Tramadol after wk 1 but pain was unbearable - especially at night. Now 2 weeks post surgery and pain is getting worse, did initially improve, but last 2 days have been really painful. What would be next step up (excuse pun) above Tramadol. When I had knee op last year I had Diclofenac & CoCodamol .... assume Tramadol is higher level that that combination.

Infection Signs - Post-Wisdom Tooth Surgery? I already posted a question about this and was doing fine for a little while after my post but am now experiencing some pain. What are some signs of infection after wisdom tooth surgery? I had all 4 impacted wisdom teeth removed 3 weeks ago. Dry sockets possibly in all four. Have been feeling better in the last 1.5 weeks but woke up yesterday in pain from clenching. My face, cheekbones are hurting radiating a little to my ears. I took 400 g of ibuprofen which has helped. The pain is intermitten and in pangs here and there. I always make sure to rinse with salt water several times a day and use the syringe. Any ideas as to whether this is healing pain or infection pain or something else? Thanks so much for all the answers and responses. I looked in my mouth and don't see any oozing - but what exactly would that look like? the gums look fairly pink and healthy on the bottom and I can't see up top obviously. There is a bit of redness on a small part of the gum area and the left gums are a bit more swollen than the right - but the surgery on the left side was much more traumatic. The swelling on my face isn't that much - there is a slight bit left (I think) and I don't feel like i have a fever. Mostly, there is pain, not in the sockets as much as pangs of pain near my ears - pangs or stabbing pain that comes and goes and is not that intense but the worse than it's been in the past two weeks. I am worried about infection because I have noticed lots of tension in my shoulders, head and neck as well and heard that infection in the mouth can travel to the brain. I am very concerned! Any other thoughts?

gouldgirl2002 replied: ""Healing pain?" No! This is probably infection. It's hard to fight infection in the mouth and you had a lot of work done. Your main sign of infection is going to be pain. You can't really go by redness because it's in your mouth."

science teacher replied: "It can take a couple weeks to get over an extraction and longer with impacted and dry sockets. Do you see any oozing? Do you have a fever? What is the swelling now? Is your face swollen?You can take 800 on the motrin. You can also take tylenol at opposite 4 hours for pain(Ty at 2, Ib at 4 Ty at6 Ib at 8. ) That is not for a long period but for severe pain before you can get to see a professional. Keep rinsing, even more often and look for oozing or swelling, redness around the area and fever."

nicealbanyguy replied: "Since you are 3 weeks post-surgery, I would become suspicious that you have some thing lodged in one of the hole left by your wisdom tooth. If you have a water pic use that to flush out each wisdom tooth hole. If you don't have a water pic then I would go to the dentist and have him flush out the holes and he will be able to tell if you have developed an infection. I would consider it unusual for you to be 3 weeks post-surgery and have pain from healing. It is either some thing stuck in there or an infection."

Post Gallbladder surgery pain in lower abdomen for 10 months!? A family member had a laparoscopic gallbladder surgery 10 months ago. Since then he has had pain in his lower abdomen below his navel that he describes as feeling like a 'sunburn' on the inside. This pain is noted during any movement. He's had a scan of the area and a colonoscopy that turned out negative. He feels that the CO2 used to inflate his belly may have stretched the area, even may have torn it, and caused this pain. Could he still have CO2 in his belly? Could he be allergic to the CO2? His family doctor had him take 2 ibuprofen 3 times a day for a week. This helped reduce the pain but did not eliminate it. Has anyone else experienced this or have an idea as to what the problem is? His surgeon says he can't help him unless he wants surgery. He returns to his family doctor in a week. Thanks!

raybdog replied: "CO2 would not still be there, but certainly something else might have made its way in there and wasn't removed. His surgeon wants to do surgery to figure this out? Whoa, big surprise there, huh? I guess the idea of doing an MRI must be considered "sissy" amongst surgeons."

Charles H replied: "Possible scarring between bowel loops from the stretching? (adhesions) The CO2 is long gone. He may have needed to move around more just after the surgery. That is when scarring can occur."

April replied: "The CO 2 for sure is gone, so that is not a consideration. but 10 months for continued pain is really unusual.... few have pain that long when gall bladders were taken out with huge incisions.... My dad has his done in the 40's when that was the only way it was done. I had a lap gall bladder about 6 years ago, and was up the next day, and would have walked to the car if they would have let me. Have had no, zero pain since, and none during the few days it took to recover. Sometimes what happens in any surgery is that there begins a pain cycle.... real pain, real nerves and really awful, despite the fact that the patient is out, and the area infused with lidocain or something similar. I'm guessing that your family doc is thinking that that might be this pain cycle problem... i.e.,, he is trying to break what he believes is a pain cycle. I think your family doc ought to call up the surgeon and find out how that surgery went.... and in some states your family member has the right to see what the surgeon wrote on the write-up, which is in the hospital files. There are some things as well that might have occurred while the surgeon was in there.... in a really inflamed gall bladder, there can leak out some bile.... it may still be leaking. And if the surgeon notes that while doing the surgery, they usually have to go in and make the larger incision. After 10 months, though, truly the pain ought to be gone, and your family member needs to have a heart to heart with someone and get some answers. And as one of your contributers noted, adhesions are certainly something that could have occurred."

When do you know it's time to run to the emergency room for an anastomotic leak from rectal surgery? In treatment for stage III rectal cancer. 3 wks post op, have been having pressure rectally, chills, HA. Told Oncologist. Tested blood WBC 12,000. I told him, Infection, he told me -allergies. 2 days later. Felt faint in shower kneeled down (250cc of drainage came out rectally). Told surgeon - have fever when not on Ibuprofen, on have low temp. Surgeon sees me & tells me, I have a small leak, not an abcess. Next day I go for chemo & surgeon tells chemo doc that I have a leak & an abcess which was drained. I'm on anti-infectives, but still have pressure pain. Is not better yet or has gotten worse. Confusion is s/s of sepsis, peritonitis, chemo, abcess, & leakage is the same. I do have more pain & s/s if I don't take ibuprofen. What to do so I don't die cause of medical stupidity or seem like a crazy lady running to the ER for nothing? Thanks for ranting....

Diesel replied: "Wow! That sounds painful. Right! Let's start by talking about Early signs of Anastomotic leakage first. This will present itself with bleeding rectally (bright red blood), especially after passing stools. On some occasions you may witness mild bleeding without you having gone to the toilet. Early signs are prior to the onset of infection. The second degree signs of Anastomotic leakage occurs after the onset of infection, eg peritonitis. This is a serious stage though and needs serious attention, or hospitalisation. The better management of this problem is to prevent it before it occurs. This is achieved by keeping your stools (bowel contents) soft all the time. * Drink as much fluids as possible (minimum of 2 Lt a day) * Avoid spicy foods * Avoid gas-forming foods eg cabbage * Get stool softners (eg Lactulose) from your GP or Pharmacy * Avoid constipating medicines if possible eg co-codamol * Also, avoid Aspirin or any aspirin-containing drugs as they make bleeding worse. * Going to the toilet a bit more often is recommended as this lessens chances of getting costipated."

How do I know if my dental bone graft is healing properly or if it has failed? I was born without a number of permanent teeth, luckily all in the back of my mouth. In order to prepare for four dental implants, my oral surgeon said I needed to get bone grafts in those four areas due to the lack of supporting bone. I had the procedure done Monday, August 17 (nine days ago) in areas 3, 14, 19, and 30 with bovine material. I was told I would be able to return to work by Friday after having some pain and maybe swelling. The remainder of surgery day wasn't bad, groggy from the anesthesia and I iced my face for 30 minutes every hour or so the entire day, not rinsing my mouth or spitting as told. The day AFTER surgery, the pain was worse- which was expected. And the third day, I woke up in excruciating pain with my face completely swollen to my eyes and my right eye bruised and red. I could also finally open my mouth enough to look at the graft sites and noticed that the sutures on one spot had already disintegrated (after only three days) and it seemed to be open. I called my oral surgeon and he said to come in the following day. When he looked around my mouth (somewhat briefly) he said that everything was "healing beautifully" and that the amount of swelling and pain I was experiencing was not uncommon. He said the swelling should subside within a few days, and the pain (and rotten taste in my mouth) within a couple of weeks. Each day since then, the swelling has completely vanished but the pain is still excruciating. I have been taking 800mg of ibuprofen prescribed to me every 6 hours on the minute- even having to wake up in the middle of every night to do so. I also noticed that the original graft area, where the sutures disappeared first and concerned me because it looked like an open hole, still looks that way. And that now three out of the four graft areas are actually like that, looking like big open holes. The past three mornings, I have also woken up with a very small amount of tiny crystal-feeling particles in my mouth, which makes me think that the grafting material is actually escaping the unclosed sites. Plus, whenever I rinse with warm saltwater- which I do 5 or more times a day (whenever I eat or drink anything besides water) since day two- the pain is 100x more excruciating and feels like it is stinging all the way under and around the teeth surrounding the graft sites, especially on my right side. I know the rinse is supposed to sting a tiny bit, but it's nine days after the procedure and it actually gets MORE painful instead of less. I am a very healthy 26 year old. I do not smoke or drink, the teeth I do have are very healthy and big, I am never sick and never one to worry about health issues irrationally or have a low tolerance for pain. I have followed the exact protocol I was given to follow post-surgery and during the healing process. And I am honestly very worried that these bone grafts have, for some reason, failed- which is so discouraging for so many, many reasons.. I have searched and googled for hours trying to find some information on dental bone grafts and the healing process, especially photos showing what it should and shouldn't look like.. but I cannot find ANY website with any helpful information or photos, which is really surprising. I made another appointment to see the oral surgeon tomorrow but figured I would try this route in hopes that someone who has gone through the experience, or even better, someone in the field of dentistry or oral surgery, could give advice and maybe even ease my mind or provide websites with information and pictures. I have taken pictures of my bone graft sites and posted the links below. Again, this is nine days post-op dental bone grafts with disintegrating sutures and bovine material. I appreciate anyone who has advice, who has gone through this, who may be a dental professional... Does it seem like the grafts have failed and I am not healing properly? And why would this happen if I have followed every protocol I was given perfectly and have been exceptionally careful? Also, if it did fail, do I still have to pay for the procedure? Would I need ANOTHER try at this or are there other, better options (because I do not want to go through this again- not to mention, I can't afford to miss work like this again)? I greatly, greatly appreciate it. Thank you. Photos (yes, I also have braces in order to make room for the grafts/implants)... I had an Apicoectomy with barely any post op instructions??? I had an Apicoectomy (otherwise known as torture!) yesterday a.m. and all they told me was ice it and no hot/spicy foods. I didn't get antibiotics although I took them a couple of weeks before the surgery. I did ice all day yesterday but woke up the next day super swollen. I read that warm moist compress may help or should I still be icing? Should I worry abt. infection and what abt. cleaning my teeth? I'm taking 600mg ibuprofen, every 4-6 hrs. can I mix that with "hydrocodone"? Would appreciate advise from any pros out there!

oz_dreamer2001 replied: "You can take hydrocodone with ibuprofen, just so you know it has Tylenol in it so the combo will help the swelling more than the ibuprofen alone since both Tylenol and ibuprofen are anti-inflammatory meds. plus the hydrocodone is a good pain med. you already took your antibiotics, so don't worry about that right now. call your dentist about cleaning your teeth, and continue with the ice as that helps swelling more than heat."

Dr.Parveen Chopra replied: "Please relax....dont worry !! As a matter of fact there is no cause of any worry. Please don't do any ice-packs anymore....these are only useful only for a couple of hours after the surgery. Yes, please take Ibuprofen......600mg if your doctor has told you about that. Otherwise, 400mg taken every 8 hourly is good enough. Your dentist has chosen not to give you antibiotics....there might definitely some reasons for that. So, just follow what your doctor has advised. Dont mix it Ibuprofen with Hydrocodone......it is not required. Yes, you can start doing warm compresses....these will be very useful. And also please do start warm saline gargles every 2-3 hours....you will find these very soothing. Dont worry much about swelling....it is bound to go in coming 2-3 or 3-4 days. So, just take it easy. Please use Betadine gargle during this healing phase for 2-3 times a day.....out of which once should definitely be at the bedtime. And still wonder what about oral hygiene........I feel you must use soft toothbrush for 4-5 days....and if tooth-brushing is just not possible for swelling or due to some other reason, just do use Chlorhexidine mouthwash for 2-3 days ..ya, as a substitute for tooth-brushing. And one very important thing....eat well-balanced nutritious diet and stay away from all sorts of junk foods. Yes, definitely not spicy stuffs at least for a week.....by which time, generally, healing of the oral wound is OK. So, smile and be happy!!"

Severe throbbing pain after wisdom teeth extraction? I had four impacted wisdom teeth removed on December 21st. In my opinion i took complete post operative care after the surgery. Followed my penicillin and anti swelling medication on schedule. Rinsed my mouth after eating any type of food. Got check 2 days after the surgery for swelling . Surgeon said i was making a splendid recovery. It is now the sixth day and i am having severe throbbing pain in my right side of the jaw. Non responsive to ibuprofen and percocet which i was prescribed. Checked for dry socket and it doesn't look like i have any. Any ideas how to deal with this till tommorow?

grandpa walleye replied: "I'll bet you have gotten a dry socket, especially if the percocet is not doing any good. There is not much relief you will get until the socket is packed with a medicated dressing. Try a percocet with an ibuprofen and see if that helps until tomorrow. good luck."

what should i do to stay calm?? Hi everyone, Three weeks ago I had a lumbar spinal fusion (for a slipped vertebrae). Today I went for my check-up with my surgeon, and I had alot of symptoms that I was concerned about. He thought I might have post traumatic stress. He said that he understood because I’m only 14, and found out I needed this surgery about 2 weeks before the actual surgery day. He said it should go away within a few weeks. The surgery went great, but I’m scared of something else might go wrong with my body again. This is mostly because I’m experiencing symptoms that he doesn’t know why their occurring (including shoulder numbness, and burning in my feet, and twitching). He thought that these symptoms might be occurring because of one of my medicine I was taking. So, he took me off of the medicine (now im only take ibuprofen, which is causing major back pain). Do you have any ideas that will help me calm down until then? (He told me he was going to speak with the neurologist that helped with my surgery, so I need help until he gets back with me…I need sleep!) Jayne S replied: "One good method to help calm yourself is to focus on your breathing. You don't need any medicine or equipment of any kind. Just pay attention to your breathing. This can actually take up a lot of attention so it tends to take your mind off the things that are bothering you. Try to breathe deeply and regularly. It should help."

what are some of the negative side effects of having a molar removed? on thursday morning, i had to have one of my molars (#19) removed due to continual problems. i was given an antibiotic and pain reliever along with post op instructions on what to do after surgery. i've been doing as the instructions say, but unfortunately i'm not feeling relief. i've noticed that i can hardly open my mouth, and for the past 2 days, i've had a horrible taste in my mouth almost like a rotten, decaying taste. i do have some swelling on that side, and i've also noticed a lump underneath the extraction site. i forgot to ask the nurse for a copy of the papers i had to sign prior to surgery, these included possible things that could happen, and i remember being concerned with only one; dry sockets which i have experienced before. i'm planning on calling my oral surgeon tomorrow, i don't think what i'm experiencing right now is an extreme emergency. does anyone know of any other negative side effects from oral surgery? what else can the oral surgeon do? in addition to my rx meds, i'm alternating ibuprofen/tylenol and doing the salt water rinses, and as of today, using warm compresses. any advice/suggestions would be very much appreciated. thank you.

LX replied: "It can cause other teeth to move around it, and into its space, it can cause your bite to shift as these are anchor teeth."

Should I call the Dr.? I had a laparoscopy 8 days ago. I had endometriosis adhesions removed. I was told it wasn't bad. I also had one tube tied since the Dr. couldn't count to two when she did it 6 months ago. We found this out during the surgery. Anyway..I was given Dilaudid 2 mg. and Ibuprofen 800 mg. The Dilaudid was supposed to be taken for a few days..one every 4 hours. I am still hurting bad. I have had to take 2 Dilauded at a time. The ibuprofen does nothing. Today I have been having bad cramps (like menstrual cramps). I took Ibuprofen first. It didn't touch it. Then I took 2 Dilaudid. I am better, but am still in pain. Is this normal 8 days after surgery? Should I call the Dr.? I haven an appt. on Mon. for my post op. I don't take narcotics normally. They make me very sick. I can't even take Vicodin. That is why he gave me Dilaudid. I take a lot of Ibuprofen. They told me not to lift over 10 lbs. for 3-4 weeks. I think it was supposed to say 3-4 days. I have a 6 month old an a disabled 21 month old. I didn't lift my 6 month old for at least 4 days. I had him brought to me. I lifted my 21 month old after about 5 or 6 days. I haven't been doing much at all lately. Kady Ann- I had abdominal cramps for 8 months. It would get really bad right before my period. I would also get diarrhea right before which I just found out is a symptom. Bloating is a symptom too.

nursekuba replied: "I would call the doctor in the AM. Dilaudid is very strong pain medication and unless you are a habitual drug user you shouldn't be hurting that badly 8 days after surgery."

Yogi replied: "I have had two laparoscopys for endometriosis. I never take pain meds,(they make me sick) just ibuprofen. (careful.. use that strength too long and you could have sever ear ringing like I did. That made me stop it at that point.) I don't remember having intense pain that long afterward. It doesn't sound right to me. I would definitely let your doctor know. Did you do any strenuous activity too soon after surgery that might slow your healing?"

PrincipessaLHO replied: "Sorry you are in so much pain. I would definitely recommend that you call your doctor/surgeon tomorrow at 9am sharp. Sounds like you need another narcotic to help with the pain. Be careful with the ibuprofen because it can do damage to your stomach. Do not go over the prescribed amount and make sure you take it with food in your stomach. If you cannot get an appointment tomorrow and the pain is the same OR worse, go to the ER or an urgent care center."
